When you leave hospital it can be very stressful and confusing time.

There seems to be a whole host of things to do and remember in addition to coping with recuperation.

• We can offer a whole range of support services to help you cope.  This can be in the form of practical help and advice with the aim of helping you to re-adjust and become indepentent again.
  
  
• We can assess your home to see if you are able to get up and down stairs or in and out of the bath.
  
  
• We can carry out minor works to install grap rails and other aids.
  
  
• We can loan you a whole range of mobility equipment like wheelchairs and commodes.
  
  
• We can arrange to take you shopping or to the chemist or do it for you.
  
  
• We can take you to the doctors or to the hospital for check-ups.
  
  
• We can come and visit you at home to help with form filling and benefit advice.
  
  
• We can also help with domestic chores and prepare a meal for you.
  
  
• We can offer you companionship and friendship at one of our day care centres where you will be able to have a meal and recieve help with bathing.

There are a range of nominal charges for the various services.

Assessments and advice are provided free of charge.
